Health History and Immunization Forms Requirement
Registering for classes and confirming housing assignments cannot be done until health history and immunization records are submitted.
After you submit your Admission Acceptance Form, you will be able to log into the patient portal at suhsportal.syr.edu, submit your health history and upload your immunization/physical form prior to arriving on campus. Community Wellness Requirements
Community Wellness Requirements are centered on sexual and relationship and violence prevention, alcohol and other drugs and diversity, equity and inclusion. Prior to arriving on campus, students will receive emails containing next step instructions for completing mandatory presentations and mandatory EVERFI Online Training Modules. These requirements will be due prior to arriving and throughout the first few weeks on campus.
Please note that failure to complete these requirements will result in a Conduct Hold.
Mandatory Health and Wellness Fee
A $375 per semester health and wellness fee is mandatory for all full-time students, and is included with billing from the bursar’s office. Part-time students are eligible to use Barnes Center at The Arch Health Care on a fee-for-service basis. The health and wellness fee does not cover all expenses such as pharmacy, laboratory, and referrals to specialty service providers. Please bring a copy of your insurance card when you visit Barnes Center at The Arch Health Care.
Student Health Insurance Enroll/Waive Form Requirement
Each year, all full-time, matriculated undergraduate, graduate and law students must enroll in or waive out of the Student Health Insurance Plan through their MySlice account. Online students are not eligible to enroll in or required to waive the Student Health Insurance Plan. Prior to arriving to campus, students will receive emails containing next step instructions for enrolling into or waiving out of the Student Health Insurance Plan. Students within traditionally online degree programs are not eligible to enroll in or waive the Student Health Insurance Plan.
Health Insurance Coverage
All incoming full-time students and returning international students are required to carry health insurance that is compliant with the Affordable Care Act (ACA) and provides coverage for services received in the Syracuse area. You may already be covered by an ACA-compliant insurance plan, and will therefore be able to waive participation in the University-sponsored plan.
If you are not already covered, you’ll be required to participate in the University-sponsored student health insurance plan.
