Health Form Submission
As part of your enrollment process, log into the patient portal at suhsportal.syr.edu, submit your health history, upload your immunization record and complete your tuberculosis screening form by July 1, 2019.
The deadline for immunization submission is July 1, 2019. Note: if you are intending on living on campus, you will not receive your housing assignment until your immunization record is received. If you are accepted after July 1, your immunization records must be submitted prior to your arrival on campus. You will not be allowed to register for classes until it is received.
Proof of immunity to measles, mumps, and rubella (MMR)
This information may be obtained by contacting your high school or primary care physician. Immunization records must be uploaded to the patient portal at suhsportal.syr.edu. Required proof of immunity includes; Documentation of two doses of live MMR after your first birthday (must have been given at least 28 days apart), positive titer results, or physician documentation of disease. (Note: Persons born before January 1, 1957, are exempt from the measles, mumps, and rubella requirement.)
A completed meningococcal meningitis vaccine response form
This must be provided to Health Services indicating that you, your parent, or guardian has received and reviewed meningitis information, and that you have either been immunized within the preceding 5 years and after the age of 16 or sign a waiver declining immunization. Information on meningococcal disease as well as the meningitis waiver form can be found online through the patient portal at suhsportal.syr.edu.
Syracuse University is obligated to enforce student immunization requirements as defined by New York State public health law. Enrolled students found not to be in compliance will be unable to register, obtain an I.D. card, and may be required to leave campus. Students that are intending on living on campus, will not receive a housing assignment until your immunization record is received.
Health Insurance Coverage
All incoming full-time students and returning international students are required to carry health insurance that is compliant with the Affordable Care Act (ACA) and provides coverage for services received in the Syracuse area. You may already be covered by an ACA-compliant insurance plan, and will therefore be able to waive participation in the University-sponsored plan. The waiver will be available in the spring of 2019.
If you are not already covered, you’ll be required to participate in the University-sponsored student health insurance plan. The cost for the plan for Fall 2019 will be available in Spring 2019.

Mandatory Health and Wellness Fee
A $375 per semester health and wellness fee is mandatory for all full-time students, and is included with billing from the bursar’s office. Part-time students are eligible to use Syracuse University Health Services on a fee-for-service basis. The health and wellness fee does not cover all expenses such as pharmacy, laboratory, and referrals to specialty service providers. Please bring a copy of your insurance card when you visit Health Services.
