Health Services provides high-quality wellness services and ambulatory health care.

Conveniently located on Waverly Avenue, Health Services will help you manage your health with office visits, pharmacy services, nutrition counseling, medical transport, and much more. You can review a full menu of student services on the Syracuse University Health Services website.

Health Form SubmissionLink

As part of your enrollment process, log into the patient portal at suhsportal.syr.edu, submit your health history and upload your immunization/physical form by July 1, 2017.


Immunization RequirementsLink

Please provide the following prior to your arrival on campus:Link

  1. Proof of immunity to measles, mumps, and rubella (MMR)Link

    This information may be obtained by contacting your high school or primary care physician. Immunization records may be faxed directly to Health Services at 315.443.9010. Required proof of immunity includes: Dates of two doses of measles vaccine, one in the form of an MMR; positive titer results, or physician documentation of disease. (Note: Persons born before January 1, 1957, are exempt from the measles, mumps, and rubella requirement.)
  2. A completed meningococcal meningitis vaccine response formLink

    This must be provided to Health Services indicating that you, your parent, or guardian has received and reviewed meningitis information, and that you have either been immunized within the preceding 10 years or have opted not to obtain immunization against meningococcal disease. Information on meningococcal disease as well as the meningitis response form can be found online through Health Services.

Syracuse University is obligated to enforce student immunization requirements as defined by New York State public health law. Enrolled students found not to be in compliance risk strict administrative consequences, including the inability to register and suspension of I.D. card privileges, and may be required to leave campus.


Health Insurance CoverageLink

All incoming full-time students and returning international students are required to carry health insurance that is compliant with the Affordable Care Act (ACA) and provides coverage for services received in the Syracuse area. You may already be covered by an ACA-compliant insurance plan, and will therefore be able to waive participation in the University-sponsored plan. The waiver will be available in the coming weeks.

If you are not already covered, you’ll be required to participate in the University-sponsored student health insurance plan. The cost for the plan for Fall 2017 will be available in Spring 2017.

Mandatory Health and Wellness FeeLink

A health and wellness fee is mandatory for all full-time students, and is included with billing from the bursar’s office. Part-time students are eligible to use Syracuse University Health Services on a fee-for-service basis. The health and wellness fee does not cover all expenses, including pharmacy, laboratory, and emergency room care, referral to specialty service providers, and hospitalization. Please bring a copy of your insurance card when visiting Health Services.
